The Synergee Podcast is a functional medicine show hosted by Kelly Engelmann, FNP-BC, founder of Enhanced Wellness Living in Ridgeland, Mississippi, with co-host Lori Esarey. Certified family nurse practitioner with more than 25 years of experience, a Master’s in Nutritional Medicine from the University of South Florida, and board certification in anti aging and restorative medicine. She founded her own clinic in 2006 on the principle that nutrition can manage disease.

What patients actually ask us

These come up in the clinic and on the show, over and over.
Why am I still exhausted when my labs came back normal?
Because normal is not the same as optimal. Standard bloodwork is built to catch disease, not to explain why you feel bad. Chronic stress, nutrient depletion, cortisol imbalance, and thyroid changes can drive fatigue for years before anything crosses into an abnormal range. We look at where you sit inside the range, not just whether you are inside it.
Yes. Stress is not only a feeling. It is held in the nervous system, the hormones, the gut, and the immune system, which is why long term stress shows up as fatigue, digestive problems, thyroid changes, and hormone imbalance in people who are otherwise doing everything right.
Cortisol is your main stress hormone and it is rarely tested properly. When it is out of balance it sets off a chain reaction. Fatigue, brain fog, weight that will not move, and broken sleep are all commonly downstream of it. Cortisol and blood sugar are closely linked, so they are usually worth looking at together rather than separately.
For the right person, at the right time, in the right amount, they can help. The risk is treating them as a quick fix. Without proper testing, body composition tracking, and clinical guidance, people lose muscle alongside fat and end up nutrient depleted. The real question is not whether they work. It is what needs to be in place around them.
We start with your history. Testing is only about twenty percent of the picture. The other eighty percent is your story: what your symptoms are, when they started, what you have already tried, what helped, and what made things worse. That is what tells us which test is worth your money and when to run it. We would rather test than guess, but the right test at the wrong time is still a wasted test.
Three different things. A food allergy is an immediate immune reaction, found through a skin prick or a blood test. A food sensitivity is a delayed reaction that can appear up to 72 hours after you eat something, which makes it very hard to trace back. An intolerance means your body is missing the enzyme to break a food down, the way lactose intolerance works. Each one needs a different plan.
We would rather you did not. The test is often fine. The problem is timing and interpretation. Antihistamines and steroids can make results falsely negative. An illness in the two weeks beforehand can make them falsely reactive. Even a colonoscopy prep will skew a panel, because it clears out the gut bacteria the test is reading. A result nobody can interpret usually leads to cutting out far more food than you needed to.
No, and this is one of the most common and most costly mistakes we see. Food sensitivities are usually a symptom of a damaged gut lining rather than the root problem. Remove the foods without repairing the gut and the list simply comes back six months later with different foods on it. We have seen a patient cut every fat from her diet based on a panel and develop a severe fatty acid deficiency and a full body rash. Nobody is meant to live on an elimination diet.
No. You do not have to pick a side. We work alongside your other providers. Many of the tests we use simply are not in a standard practice’s toolbox, which is not the same as your doctor holding something back from you. Our job is to add the pieces that are missing, not to replace the care you already have.
